ABIM MOC

In this online ABIM-approved Performance Improvement activity, physicians can develop effective
diabetes management regimens using office-based support materials and systems, with the ultimate goal
the improvement in the health outcomes of their patients with diabetes. This activity is a component of the
Joslin Professional Education Continuum (JPEC), a multi-stage program of educational activities
supporting performance improvement.

Development of Office-based Systems of Care: Type 2 Diabetes and Metabolic Syndrome

Most people with type 2 diabetes in the US are not at goal for glycemia and other comorbidities,
including hypertension and dyslipidemia. This PI activity will provide physicians with the tools to assess
their practice, to choose appropriate educational and office system interventions to the improve care
of their patients with diabetes, and to measure the results.

To Participate

  1. Go to the JPEC Type 2 Diabetes homepage and register as a new member
    or log in if already a current member of JPEC.
  2. Click on the Type 2 Diabetes Performance Improvement link.
  3. Follow the steps in the activity. The minimum duration for participation is 4 months,
    allowing enough time for the selected interventions to be effective.
  4. After completing the activity, you will receive immediate CME credit.
    Follow the instructions below to receive MOC credit.

Instructions to Physicians for Reporting Participation
in an AQI Pathway Project to ABIM

In order to receive credit, you must:

  • Be a physician enrolled in the ABIM MOC program
  • Provide direct or consultative care to patients as part of the activity
  • Implement interventions to improve care as guided by the activity
  • Collect, submit and review relevant clinical data
  • Meet minimum duration criteria (4 months)

After completing the Development of Office-based Systems of Care: Type 2 Diabetes
and Metabolic Syndrome,
you can attest to your participation by submitting a report to ABIM
through your Physician Homepage on http://www.abim.org/. Simply log in using your ABIM
ID and password and click “Report Continuous Quality Improvement activity for practice
performance credit” listed under “My Maintenance of Certification Program.”

MOC credit
